• Follow-up telephone consultation for an existing patient (30 Minutes) $150. The telephone consultation must be paid in advance. The price includes a prescription, if judged necessary by the doctor, which can be sent to your pharmacist or to you directly. You must have had a medical examination with our doctors within the past 3 months. Additional amount for each 10 Minutes $ 50.00

  • 100% reimbursement of consultation fees, if the appointment is canceled before 9:00 am on the day of the appointment. Please note: the access fee of $95+tx is non-refundable
  • No refund of consultation fees nor access fees, if the appointment is cancelled after 9:00 a.m. on the day of the appointment.

• Medication renewal without a new medical consultation (only for patients seen within 3 months of a physical exam by our doctor) $40. Please not that the doctor may refuse such a request at no charge.

• Medical assessment of an incapacitated person for the purposes, for example, of the homologation of a protection mandate $480

• Disability parking permit application (SAAQ) $100.00
• Application form for exceptional medication (RAMQ) $100
• Application form for a photo or signature exemption (RAMQ) $100
• Application form for tax credit for disabled persons (CRA) $100
• Certificate respecting an impairment (Revenu Quebec) $100
• Medical note or certificate (school absence, summer camp forms and such) $100
