The nurse and you
Follow-up and management of the following chronic diseases:
- High blood pressure (hypertension)
- Type 2 diabetes
- Cholesterol
- Alzheimer and other forms of dementia (neurocognitive disorders)
Wound care :
- Wound assessment (surgical, traumatic, pressure ulcer, acute/chronic)
- Change of dressings, request for services at the CLSC, teaching of the wound care
Identification of neurocognitive disorders, recommended if a loved one, a member of your family, a friend or yourself show :
- A memory disorder
- An abnormal change in personality, behavior and/or mood
- Loss of autonomy or new confusion
The respiratory therapist and you
Spirometry test (FEV) :
- Recommended for people with asthma, smokers or ex-smokers
- The earlier spirometry is done, the earlier chronic obstructive pulmonary disease (COPD – emphysema/chronic bronchitis) can be detected and treated
Teaching patients with living with COPD or asthma::
- The respiratory therapist informs you about treatments to alleviate symptoms, ways to reduce flare-ups (exacerbations) and improve your daily life
Review of the way you take your inhalor :
- Review of your method of taking medication, to make sure it is well adjusted to your inspiratory strength
